Diabetes Prevention

Hmong American Partnership is offering a lifestyle change program for participants who are at high risk for getting Type 2 diabetes or are pre-diabetic. The lifestyle change program uses an evidence based curriculum developed by the Center for Disease Control and Prevention. This program has proven to help prevent or delay the onset of Type 2 diabetes by teaching participants the necessary skills to make healthy life style changes.

This program is one year long and contains 16 weekly, four biweekly, and six monthly meetings. The meetings are led by a trained lifestyle coach who will facilitate a group of adults that are working towards a similar goal. The goal of the program is to help the participants make small changes to lose 5-7% of their body weight and gradually gain 150 minutes of physical activity.


  • 18 years and older
  • Body Mass Index (BMI) of 22 or higher
  • Pre-diabetic or high risk for developing Type-2 diabetes
