The LIFE program helps Elders connect with community and available resources. LIFE program staff help Elders to navigate systems and receive the kinds of support that help them gain and maintain needed community services and support. Some of the services this program provides to Elders include: home visits to help them complete paperwork, maintain benefits, read letters, set appointments, arrange for transportation and interpretation services, etc.

This program is available to Elders as authorized by the Elder's HMO care manager.

Criteria/Eligibility:

• 60+ years old
• MSHO or Elderly Waiver recipient
